Birth and Death Certificates
Certified copies of Birth and Death Certificates can be requested for individuals who were born or who died in Mountain Lakes. To request a certified copy, use the form here. This is a multi-purpose form used by the Borough for any request for a Certification or a Certified Copy of a Vital Record. When you fill it out, please be sure to indicate exactly what you are requesting. Fax, mail, or bring it in to Borough Hall. If you mail it, please be sure to include the required fees. If you fax or bring it in, you will pay the fees when you pick up the documents.
Marriage and Civil Union
If you are planning on entering into a marriage or civil union in New Jersey, there are a few things you need to know…
First, if you currently live in New Jersey, you must apply for your license in the municipality in which either applicant currently resides. If neither applicant is a resident of New Jersey, but you are having your legal ceremony in New Jersey, you apply in the municipality in which you are planning to be married or enter into a civil union partnership.
You must apply for your license in person. Both the applicants must be present, as well as one witness at least 18 years old. You must present proof of identification, and if you have dissolved a previous marriage or civil union, you must present legal papers.
After you have applied, there is a 72-hour waiting period before the license can be issued. You also should be aware that the license is only good for 30 days from the day of issuance, so you must plan accordingly.
If you are a resident of Mountain Lakes, or a resident of a different State, but are planning on entering into a marriage or a civil union partnership in Mountain Lakes, please phone ahead for an appointment to apply for your license. Registrar’s hours are Monday-Friday, 1 p.m. to 4 p.m.
Once you are united in marriage or a civil union partnership, you should have at least one certified copy of your Marriage or Civil Union certificate. This certificate can only be issued by the municipality in which the legal ceremony was performed. Certified copies are available in Mountain Lakes for a cost of $15 each.
